CARES Act Provider Relief Fund

- CARES Act Provider Relief Fund – Terms and Conditions
Under the CARES Act Provider Relief Fund, the provider is required to sign the Terms and Conditions in order to retain this stimulus payment. The attestation must be made within 30 days of receipt of payment, which for most providers will be May 5, 2020. In order to sign the attestation, you will need your TIN associated with the Medicare Provider Number, the last six digits of the deposit account number and the amount of the Relief Fund Payment. Be sure to read the entire Terms and Conditions document in order to agree to all terms. The portal to sign the attestation statement has been setup today and can be found here.
